• The bakery area hand-washing sink was blocked. The dish machine’s sanitizing cycle was not hot enough. A spray bottle of glass cleaner was stored improperly above food in the meat-cutting room. The pH meter for testing acidified white rice preparation was broken.

•Foods in the cook line’s two-door reach-in cooler were holding at unsafe temperatures. The three-bay sink was used for thawing food under running water despite having no backflow prevention in its drain plumbing.

Indian Jewel, 6711 Airport, Holland, inspected Sept. 7.

• The ice machine’s interior was dirty. Refrigerated foods lacked required date marks.

Stone Oak Country Club - Pool, 100 Stone Oak, Holland, inspected Sept. 8.

•No person-in-charge qualified in food safety was present during inspection. The dish machine failed to dispense chlorine sanitizer. Hot dogs, sour cream, and macaroni and cheese in the merchandiser cooler were holding at unsafe temperatures. Working chemical containers lacked labels.

Stone Oak Country Club, 100 Stone Oak, Holland, inspected Sept. 8.

• An employee failed to wear gloves while cutting cucumbers. Raw fish and raw beef were stored improperly above sauce and milk in the cook line’s cooler. The kitchen and downstairs bar dish machines’ sanitizing cycles were not hot enough. The slicer was dirty. Refrigerated foods lacked required date marks. Gnats were present.

Penn Station East Coast Subs, 1266 S. Holland-Sylvania, Holland, inspected Sept. 8.

• Chemical spray bottles lacked proper labels. Gnats were present.

Gyro & Kabob Express, 7130 Airport, Holland, inspected Sept. 8.

• A problem with the hand-washing sink made it complicated to use. Par-cooked chicken in a pan on the steam table was holding at an unsafe temperature. Cooked gyro and soups were hot-holding at unsafe temperatures. Refrigerated foods lacked required date marks.

Firepit Grille, 7723 Airport, Holland, inspected Sept. 8.

•Facility was vacuum-packaging foods without a required safety plan. Facility was improperly using reduced-oxygen packaging for fresh fish without a required safety plan.

LaVie Mediterranean Restaurant, 1572 Spring Meadows, Holland, inspected Sept. 13.

• Raw chicken was stored improperly above raw beef and ready-to-eat soup. The juice machine’s interior was dirty. Refrigerated foods lacked required date marks. Stainless-steel polish was stored improperly. Working chemical containers lacked proper labels.

White Tower, 1515 W. Sylvania, inspected Aug. 27.

• The dish machine did not dispense chlorine sanitizer properly. The can opener was dirty.

Taco Bell, 5323 Monroe, inspected Aug. 30.

•Quarternary ammonium sanitizer in the cook line wipe-cloth bucket was too weak, and no sanitizer was set up in the front area.

Kentucky Fried Chicken, 6790 W. Central, inspected Aug. 30.

• The hand-washing sink’s hot water temperature was unsuitable. The hand-washing sink near the office was inoperable. The chicken make station, inside of pop nozzles, and food thermometer were dirty.

Organic Bliss Deli & Bakery, 3723 N. King, inspected Aug. 30.

• Two water bottles were soaking in the hand-washing sink. The chlorine sanitizer bucket tested at 0 ppm.

BJ’s Restaurant & Brewhouse, 4905 Monroe, inspected Aug. 31.

•Smashed potatoes in the warmer were holding at an unsafe temperature. Cut watermelon out of refrigeration in the bar area was holding at an unsafe temperature. Cooked ribs in the walk-in cooler and cookline cooler lacked date marks.

University of Toledo Student Union - True Burger, 2801 W. Bancroft, inspected Sept. 1.

•Whipped topping left out of refrigeration was holding at an unsafe temperature.

University of Toledo Carlson Library - Starbucks, 2801 W. Bancroft, inspected Sept. 1.

• Coconut milk was holding at an unsafe temperature. Creamer was holding at an unsafe temperature.

Manos Restaurant, 1701 Adams, inspected Sept. 1.

• Employees used the hand-washing sink as a dump sink. Raw eggs were stored improperly. The interiors of all coolers and the freezer were dirty. The upstairs ice machine was dirty. Dirty food containers were stacked with clean food containers. Rice and chicken were hot-holding at unsafe temperatures, as was beef gyros meat. Gnats were present.

Mercy Health St. Vincent Medical Center - Coffee, 2213 Cherry, inspected Sept. 2.

• Parts of the deli slicer next to the oven were dirty.

Spirit, 5001 Douglas, inspected Sept. 2.

• The soda machine’s ice chute was dirty.

Rally’s, 1647 W. Laskey, inspected Sept. 2.

• Food-contact surfaces were not cleaned at required four-hour intervals. The milkshake machine was dirty. The ice cream machine was dirty. Containers were dirty.

Jing Chuan Restaurant, 4424 Secor, inspected Sept. 2.

• The Hobart mixer and noodle machine were dirty. Left over bowls were present on the prep table and make line. A dirty flour pan was used for chicken and other food.

Hola Authentics, 3320 Glendale, inspected Sept. 2.

• An employee washed his gloved hands instead of removing them. Cuban sandwiches sitting out of refrigeration and pork and beef in the walk-in cooler were holding at unsafe temperatures. Items in the walk-in cooler lacked required date marks.

Central Catholic High School, 2550 Cherry, inspected Sept. 2.

Quaternary ammonium sanitizer in the three-bay sink was weak.

Black Rock Bar & Grill, 5001 Monroe, inspected Sept. 2.

An employee used a hand-washing sink improperly. Food items in the walk-in freezer lacked covers. Cut tomatoes and spinach dip in the prep cooler were holding at unsafe temperatures. Cooked ribs in the walk-in cooler lacked a date mark. The facility was using reduced-oxygen packaging for soups and sauces without proper documentation/safety procedure.

Pilot Travel Center - Subway, 5820 Hagman, inspected Sept. 3.

•Lettuce and tomatoes in the two-door cooler in the back food prep area were holding at unsafe temperatures.
