Violations:

Toledo Museum of Art, 2445 Monroe, inspected Aug. 24. Employees improperly used disposable towels to line prep-top cooler pans into which greens were then placed. The ice scoop’s holding pan was dirty inside with debris. The sandwich press was dirty with buildup. Salmon, pizza sauce, and salad dressing in the True two-door freezer were holding at unsafe temperatures. Gnats were present in the dish machine area. Food containers and utensils were cracked. broken. melted, or otherwise damaged.

Toledo Museum of Art - Glass Pavilion Kitchen, 2444 Monroe, inspected Aug. 24. The dish machine did not get hot enough during the wash cycle. The facility’s hot water supply was not hot enough.

Circle K, 6775 Dorr, inspected Aug. 24. Quaternary ammonium sanitizer was too strong.

Mercy Healthcare Center Cafe, 2200 Jefferson, inspected Aug. 25. The panini press was dirty with buildup.

Marco’s Pizza, 3678 Rugby, inspected Aug. 25. Cans of mushrooms had excessive denting on rims and side seals. Overfilled toppings in pans were too warm. The food preparation sink lacked an air gap in its drain plumbing to prevent sewage backflow.

Mancy’s Italian, 5333 Monroe, inspected Aug. 25. The dishwasher lacked a sight glass or alarm to indicate low supply of detergent or sanitizer.

Stop & Shop, 3241 Dorr, inspected Aug. 26. Packaged raw bacon was stored improperly over ready-to-eat foods in a cooler.

In & Out Mart, 2007 W. Sylvania, inspected Aug. 29. The hand-washing sink was blocked by items in front of it and in its basin. Choke cleaner was displayed improperly above food items for sale. Chemicals were stored improperly above the three-bay sink. No hot water was available in the facility.

Toledo Food Market, 303 Main, inspected Aug. 29. The milk display cooler’s bottom shelf and the meat scale were dirty with buildup.

Black Rock Bar & Grill, 5001 Monroe, inspected Aug. 29. The high-temperature dish machine did not get hot enough during final rinse to sanitize wares. The bar glass washer did not dispense chlorine sanitizer when required. Hamburger patties, chicken, cooked onions, and cheese in the chef base cooler, double-panned fish filets in the prep cooler, spinach artichoke dip and andouille in the prep-top cooler, portioned cups of sour cream in the expo prep-top cooler, and portioned cups of whipped butter left out of refrigeration were holding at unsafe temperatures.

Toledo Sports Center, 1516 Starr, inspected Aug. 29. Food preparation surfaces were not sanitized prior to food preparation. Time/​temperature controlled foods were date marked improperly.

Tee Oriental Food & Gifts, 1101 N McCord, inspected Aug. 30. The back hand-washing sink was used improperly for washing personal dishes. The front hand-washing sink was used improperly for rewashing single-use items. Quail eggs were stored improperly above vegetables. Tongs used for getting pork tails were dirty. Gnats were present by fresh produce and by the mop sink.

Rice & Shine, 4939 Dorr, inspected Aug. 30. The person-in-charge could not demonstrate knowledge of cleaning and sanitizing and did not know dishware needed to be sanitized after washing. Employees were not verifiably informed of a required policy to report illness relevant to food safety. The sushi rice container cart blocked access to the sushi counter hand-washing sink. Raw fish and cream cheese in the sushi cooler were holding at unsafe temperatures. Acidified white rice for sushi was not properly marked with an expiration time. A container of bok choy was in the three-bay sink, which does not have an air gap in its drain plumbing to allow use for produce washing.

OB’s Bar & Grill, 2243 Tremainsville, inspected Aug. 30. All pizza toppings in the prep cooler had expired date marks. The ice well’s scoop had excessive limescale buildup.

Long John Silver’s, 1814 W. Alexis, inspected Aug. 30. Both soda machines’ pop nozzles were dirty inside with black buildup. Expired coleslaw dated 3/​27 had not been discarded.

Goody’s Sports Nut Saloon, 1311 S. Detroit, inspected Aug. 30. The ice machine had slight buildup on its plate. Pizza racks were dirty with buildup and debris.

Costco Wholesale, 3405 W. Central, inspected Aug. 30. Pallets of body lotion and gel hand wash were stored improperly above food.

Club House Child Care, 1824 W. Sylvania, inspected Aug. 30. Food equipment and surfaces were not being cleaned and sanitized properly.

Chinatown, 1750 W. Laskey, inspected Aug. 30. Foods lacked required date marks. Live gnats were present througout.

Zacharya’s Eatery & Cafe, 3455 W. Alexis, inspected Aug. 31. The ice machine’s interior was dirty with buildup.

Tim Hortons, 4735 Monroe, inspected Aug. 31. A food handler washed hands without removing and replacing single-use gloves. Milk in the display cooler was too warm.

The Wing Station, 4015 Secor, inspected Aug. 31. The person-in-charge failed to ensure employees properly maintained safe food temperatures during hot and cold storage. The prep-top cooler failed to maintain proper refrigeration temperature and many cooked foods holding by the grill were not hot enough. The person-in-charge failed to ensure that gyro on the spit was being cooked to the proper temperature, verified with a probe thermometer. Sliced gyro sat out in a pan at room temperature. Chickpeas, raw chicken, cut tomatoes, and gyro in the walk-in cooler and cheese, sausage, and gyro in the pizza prep-top cooler were holding at unsafe temperatures.

Pat & Dandy’s, 3340 W. Laskey, inspected Aug. 31. Deli meat lacked required date marks. Expired food items were present in the prep cooler. Multiple chemical spray bottles hanging above the three-bay sink had no labels.

Awesome Wings, 1488 W. Sylvania, inspected Aug. 31. No person-in-charge qualified in food safety was present during inspection. The owner and employees present did not know how to wash dishes properly or sanitize food-contact surfaces, and were unfamiliar with safe food temperatures. The person in charge failed to ensure employees effectively cleaned their hands. The person-in-charge was unable to state the required temperatures for safe cooking, holding, storing, cooling, or reheating food and could not demonstrate knowledge of cleaning and sanitizing. The person in charge was unable to demonstrate knowledge of the relationships between prevention of foodborne illness and employees’ personal hygiene, safe food temperatures, facility cleanliness, hand-washing, or contamination. A food employee failed to wash hands after working with raw chicken and then changing gloves to begin preparing food on the main cook line. The owner washed his hands in the three-bay sink instead of using the proper hand-washing sink. Food employees failed to wash their hands when changing gloves. Food was stored in containers that had 1-2 inches of frost ice in them from prolonged storage in a freezer and were not cleaned before use. Foods touched bags in which other foods were stored. Raw shell eggs in the walk-in cooler were stored improperly above produce and shredded cheese. A container of raw ground beef was stored above cooked gyro and chicken meat in the main cooking line’s prep-top cooler. Food was stored in containers “that had not been clean in an extended period of time.” The ice machine’s bin was dirty inside with mildew and debris and there were dead flies and black debris in the bin. The entire breading station, including pans and utensils used there, was caked in old dried bread and raw juices. The table and surrounding area were also covered in caked dried breading. The prep-top cutting boards on both of the prep-top units were extremely dirty, had a foul odor when lifted, and were covered in mildew and stains. Ice and debris had built up inside food containers inside the freezer. No equipment or utensils were being sanitized after washing. Cheesecake in the dining room’s bakery display cooler, ranch dressing in portioned cups and in squirt bottles left out at room temperature, and raw chicken wings in the walk-in cooler were holding at unsafe temperatures. The main cook line prep top cooler and the main walk-in cooler failed to maintain proper refrigeration temperature. No date marks were applied to any foods for which they are required. A nonpotable hose was attached to the food prep sink’s faucet and the sink’s plumbing had no backflow prevention device. The mop sink, where such a hose could properly be attached to the faucet, needed a backflow prevention device.
