Violations:

Arby’s, 4411 Talmadge, inspected Sept. 16. The insides of the drive-up widow’s fountain beverage dispenser’s nozzles were dirty with buildup. The dish machine did not dispense chlorine sanitizer during the final rinse. A can of whipped topping stored in a pan on ice and gyro meat and Philly cheesesteak in the prep-top cooler were holding at unsafe temperatures. Several flies were present, including some in the bottom of the broken prep-top cooler and fly eggs on the cooler’s door frame.

Applebee’s Neighborhood Grill & Bar, 4702 Monroe, inspected Sept. 16. The high-temperature dish machine was not getting hot enough to sanitize wares during the final rinse. Nacho meat and soup in the midline cooler were holding at unsafe temperatures.

Wings & Grill, 519 S. Reynolds, inspected Sept. 19. The hood system was dirty with dust, debris, and grease. Chile, feta cheese, and raw chicken in the walk-in cooler and sauces in the display cooler lacked date marks.

Twin Peaks Restaurant, 5060 Monroe, inspected Sept. 19. A trash can was stored in front of the hand-washing sink. The dish machine did not dispense chlorine sanitizer during the final rinse. Sauteed mushrooms, sauteed onions, and pimento cheese in the chef base were too warm.

J. Alexander’s, 4315 Talmadge, inspected Sept. 19. Wild rice in the small hot unit at expo was holding at an unsafe temperature. Beef, fish, and chicken in the meat cooler, salmon, veggie burgers, and noodles in the prep cooler, and shrimp, chicken, and fish in the Atosa chef base were holding at unsafe temperatures.

Souk Mediterranean Kitchen, 139 S. Huron, inspected Sept. 20. A trash can blocked access to a hand-washing sink.

L&J Turtle, 1519 Eleanor, inspected Sept. 20. Food prepared in a private home was illegally offered for human consumption. The ice machine’s interior was dirty with buildup. Time/​temperature sensitive foods in refrigeration lacked date marks. Time/​temperature sensitive foods were held past expiration dates. Flies were present throughout the facility.

McDonald’s, 7228 Dutch, Waterville, inspected Aug. 19. Burrito mix in the prep area under-counter cooler and burrito mix and Big Mac sauce in the walk-in cooler were holding at unsafe temperatures.

Kroger, 8730 Waterville-Swanton, Waterville, inspected Aug. 19. Mayonnaise and creamy wasabi sauce made with mayonnaise were left out at room temperature despite mayonnaise labeling to refrigerate after opening. Small flies were present in the deli area.

Shawn’s Irish Tavern, 105 S. Third, Waterville, inspected Aug. 29. The popcorn machine’s interior surfaces were dirty with residue and buildup. Ham and pepperoni in the pizza prep-top cooler were holding at unsafe temperatures.

Pisanello’s Pizza, 57 S. Third, Waterville, inspected Sept. 6. Employees were not verifiably advised of a required policy to report illnesses relevant to food safety. The prep area’s dough rollers were dirty with dried dough residue and its pizza slicers had greasy residue. Ricotta-pesto mixture and a container of lunchmeats were held past marked expiration dates.

Circle K, 103 Anthony Wayne Trail, Waterville, inspected Sept. 16. A trash can blocked access to the front counter’s hand-washing sink and a used pizza cutter was in the sink’s basin. The ingredients statements for the self-service baked goods case lacked allergen information. Hot dogs were thawing in the three-bay sink, which lacked proper drain plumbing for use in food preparation.

Vito’s Pizza, 5915 Weckerly, Whitehouse, inspected Aug. 25. Cleanliness of equipment food-contact surfaces and utensils. Equipment food contact surfaces or utensils are unclean. Observed build up on the cutting surface of the can opener.

Whitehouse Inn, 10835 Waterville, Whitehouse, inspected Aug. 29. Water for the dish room hand-washing sink was turned off, the cold-water faucet was stripped and not working, and a trash can blocked access to the sink. The dish machine’s chlorine sanitizer supply was exhausted. Roast beef and shredded cheese in the walk-in cooler were holding at unsafe temperatures. The salad prep-top cooler holding hard-boiled eggs in its bottom level and the prep-top cooler holding tomatoes and shredded cheese both failed to maintain proper refrigeration. Milk in the under-counter dessert cooler was holding at an unsafe temperature.

Hope United Methodist Church, 10610 Waterville, Whitehouse, inspected Sept. 1. Raw shell eggs were stored improperly above ready-to-eat foods in the two-door reach-in cooler. Ham in the two-door cooler was held past its marked expiration date.

Kentucky Fried Chicken, 7150 Orchard Centre, Holland, inspected Aug. 22. Macaroni and cheese was being reheated improperly using the hot-holding cabinet. A chemical spray bottle hung improperly on a box of honey sauce in the kitchen.

Circle K, 8154 Airport, Holland, inspected Aug. 26. The self-service doughnut/​bakery case had no ingredients information.

Wendy’s, 6525 Airport, Holland, inspected Sept. 12. The hand-washing sink near the salad cooler was used improperly as a dump sink for coffee or similar liquid.

Spicy Tuna, 7130 Airport, Holland, inspected Sept. 13. The bar hand-washing sink’s faucet handles were missing. The ice machine’s interior was dirty with buildup.

Ralphie’s Spring Meadows #2, 6609 Airport, Holland, inspected Sept. 13. Ham and turkey in the cookline two-door under-counter cooler were holding at unsafe temperatures. Sauerkraut in the cooler was held past the marked expiration date, which may have been the result of the container being refilled without a new date label.

Mike’s Pizza & More, 847 S. McCord, Holland, inspected Sept. 13. Cutting boards’ condition was unsuitable for easy cleaning.

Texas Roadhouse, 6137 Trust Drive, Holland, inspected Sept. 19. The bar dish machine was not dispensing chlorine sanitizer.

Penn Station East Coast Subs, 1266 S. Holland-Sylvania, Holland, inspected Sept. 19. The lemonade container, ketchup container, potato bins, potato cutter, and pop nozzles were dirty. Foods in the middle prep cooler were holding at unsafe temperatures. Working chemical containers lacked labels.

Chitter Chatter Ice Cream, 8144 Airport, Holland, inspected Sept. 21. Strawberry yogurt mix in a soft-serve machine was holding at an unsafe temperature.

The New Bono Tavern, 843 Main, Martin, inspected Aug. 25. Raw shell eggs were stored improperly above other foods in the two-door reach-in cooler. Portioned food items lacked required date marks.

Staybridge Suites, 2300 Village W. 1800, Maumee, inspected Aug. 26. Oatmeal in the server hot-hold unit was holding at an unsafe temperature. Cream cheese packets sitting out at room temperature were too warm. All items in the cold buffet were too warm, including cottage cheese, watermelon, and milk. Quaternary ammonium sanitizer in the wipe-cloth bucket was too strong.

Chipotle Mexican Grill, 1385 Conant, Maumee, inspected Aug. 29. An employee handled grilled chicken from the make table without wearing gloves. Grilled chicken and carnitas in the make table by the office were hot holding at unsafe temperatures. Items in the reach-in cooler were too warm. The ice machine’s drain plumbing lacked an air gap in its drain plumbing.

The Pinnacle, 1772 Indian Circle, Maumee, inspected Aug. 30. The ice machine’s interior was dirty with buildup.

Le Petit Gourmet, 6546 Weatherfield, Maumee, inspected Aug. 30. The can opener’s cutting surfaces were dirty with buildup.

Sidelines Maumee, 1430 Holland, Maumee, inspected Aug. 31. Dirty lids and squeeze bottles were stored in bins considered clean. The can opener’s blade was dirty with food debris. Containers holding clean steak knives, lids, and other utensils were dirty with food debris. The back ice machine was dirty with black/​brown buildup. Chili, chicken soup, French onion soup, and other foods were reheated improperly using the steam table. The back ice machine and bar ice bin both lacked required air gaps in their drain plumbing.

Great American Cookies/​Pretzel Time, 3100 Main, Maumee, inspected Sept. 13. Employees were not verifiably advised of a required policy to report illnesses relevant to food safety.

Walt Churchill’s Market, 3320 Briarfield, Maumee, inspected Sept. 14. Juice and protein drink in the retail bottled juice cooler and ham, cheese, and cheddar cheese in the cafe area’s drawer cooler were holding at unsafe temperatures. Turkey in the deli service cooler was held past its marked expiration date.
