Kroger, 2555 Glendale, inspected Nov. 18. The person-in-charge failed to ensure that employees cool foods properly. Temperature logs for rotisserie chicken were missing. Access to the deli hand-washing sink was blocked.
Karing 4 Kids with Creative Mindz Learning University, 4263 Monroe, inspected Nov. 18. An unapproved portable sink was in use.
Yang’s Gourmet House, 4446 Heatherdowns, inspected Nov. 21. The person-in-charge could not show knowledge about safe temperatures and times for storing, heating, cooling, or reheating foods. Raw shrimp was stored improperly below raw beef in the walk-in cooler. The can opener blade was dirty with food debris. Bullion base was in dirty containers. An employee did not sanitize a cutting board after washing and rinsing it. Shrimp was being peeled at an unsafe temperature. Cut cabbage was holding at unsafe room temperature. Cooked noodles, hand, and shrimp in the make table were holding at unsafe temperatures. Spring rolls in the two-door reach-in cooler lacked date marks. Neither the prep sink nor the two-bay sink by ware washing, which also was in use for food prep, had proper air gaps in their drain plumbing to prevent drainage backflow.
Vito’s Pizza, 4317 Heatherdowns, inspected Nov. 21. The person-in-charge failed to ensure employees wash their hands effectively. An employee failed to wash her hands after warming them in her apron and before donning gloves to work with food. The can opener blade was dirty with food buildup. Buckets of food in the tops of the make tables lacked date marks. A bucket of cooked sausage was held past its marked expiration date. A chemical spray bottle in the employee restroom had no label. The prep sink’s drain plumbing had no air gap to prevent drainage backflow.
The Onyx Cafe, 27 Broadway, inspected Nov. 21. Quaternary ammonium sanitizer in the wipe bucket tested at 0 ppm. Quiche was held past its marked expiration date.

Raw shell eggs were stored improperly above raw hash browns. Cut tomatoes in the make table were holding at an unsafe temperature. Two containers of batter lacked date marks. Gnats were present in the back-of-house area. The ice machine and server ice bin drain plumbing lacked air gaps to prevent drainage back flow.

Maurice’s Green Lantern, 509 Broadway, inspected Nov. 22. The person-in-charge could not show knowledge of the relationship between food safety and proper equipment, specifically pertaining to reusing dirty dishes. An employee used a dirty bowl under a shelf to mix eggs. French fries hot holding by a window with no heat source were not hot enough. Cut and cubed tomatoes in a pan of ice on the counter were holding at unsafe temperatures. French fries cold-holding by a window sill were too warm. Food pans with old date marks were not being washed before reuse.
Lucha Libre Taco Shop, 4635 Douglas, inspected Nov. 22. Foods were held past their marked expiration dates.
Holey Toledough, 3812 W. Alexis, inspected Nov. 22. The dish machine did not dispense chlorine sanitizer properly. Washed containers were dirty with stickers or sticker residue.
Toledo Early College, 2800 Bancroft, inspected Nov. 28. Popcorn chicken and potato wedges in the hot-hold cabinet and popcorn chicken in the service line hot-hold unit were holding at unsafe temperatures.
Ohiolink Corrections & Treatment, 2012 Madison, inspected Nov. 28. An insulated beverage dispenser had a black garbage bag as a liner for storing and dispensing ice for drinks. Bologna-and-cheese sandwiches were mistakenly placed in a two-door cooler designated exclusively for bread storage and became too warm. Cut leafy greens, lunch meats, lunch meat sandwiches, and other perishable foods lacked required date marks.
McDonald’s, 3138 Secor, inspected Nov. 28. The McCafe beverage machine’s ice bin was dirty inside with buildup. The three-bay sink’s sprayer nozzle dangled below the sink’s flood rim, creating a backflow hazard.
Jamos Pub, 3223 W. Sylvania, inspected Nov. 28. Multiple containers of food in the prep-top and walk-in coolers and in the freezer were left uncovered overnight. Thirty raw eggs were improperly stored directly on a case of avocados that was above raw beef and cooked potatoes. The ice machine’s interior slide was dirty with pink-yellow slime. The flat-top grill was extremely dirty with grease and food-debris buildup. Food storage containers in the top of the prep-top cooler were dirty and apparently were refilled without being cleaned. The prep-top cooler’s cutting board was dirty with food debris from the day before. The cutting board was stained and no longer in good condition. Oil in the fryers was very dark and had lots of floating food debris. Clean pans stored under the main cooking line were full of food debris. Time/temperature sensitive foods lacked required date marks, including cooked baked potatoes, cheese sauce, chicken Buffalo dip, cut leafy greens, opened containers of cream, and cut tomatoes. Some items in the prep-top cooler showed advanced spoiling. Items with date marks had not been discarded upon expiration. A large pile of dirty, wet rags in a bus tub under the three-bay sink had attracted a gnat infestation.
Good Times Bar & Grill, Inc., 5119 Jackman, inspected Nov. 28. Chili was held past its marked expiration date.
Denny’s, 6920 W. Central, inspected Nov. 28. No person-in-charge certified in food safety was present during inspection. Brushes were stored improperly in the hand-washing sink by the freezer. Quaternary ammonium sanitizer in the wipe bucket tested at 0 ppm. The prep salad cooler and drawer cooler both failed to maintain proper refrigeration temperature and foods in them were too warm. Oven cleaner was stored improperly in food prep areas. Gnats were present at the server station.
Carlos Poco Loco, 1809 Adams, inspected Nov. 28. No person-in-charge certified in food safety was present during inspection. Bags of produce were stacked in direct contact with open food containers in the prep-top cooler, and boxes of food were placed on top of open food. Washed and prepared pineapple halves were stored in their dirty delivery boxes. Cuban pork, masa, and refried beans were left out at room temperature to cool instead of proper, active cooling. Foods in the hot-holding cabinet were too cool. Loose pest bait was improperly used in the basement. Standing soda in the pop-gun holsters attracted gnats.
Byblos, 1050 S. Reynolds, inspected Nov. 28. A rack blocked access to the hand-washing sink. The ice machine was dirty. Hummus and a perishable sauce in a cooler were holding at unsafe temperatures. Working chemical containers lacked proper labels.
