Violations:

Graze Shared Kitchen, 1734 W. Laskey, inspected April 30. Employees did not wash their hands before starting food preparation. Equipment, food-contact surfaces, and utensils were not cleaned or sanitized at the required frequency. Both dough sheeters were filthy. The underside of a mixer and the cage area were filthy with buildup. A dirty pitcher was stored with clean items on a wire storage rack. The knife rack was dirty. The probe thermometer was dirty with caked-on buildup. Racks for the dish machine had extreme buildup. Several sauces in the walk-in and salad coolers had expired.

University of Toledo Medical Center Dietary Service - Mulford Garden, 3000 Arlington, inspected May 7. The ice machine’s drain plumbing lacked a proper air gap to prevent sewage backflow.

University of Toledo Dietary Service - Main Kitchen, 3000 Arlington, inspected May 7. A hand-washing sink was blocked. Cut lettuce in the iced well was too warm. Shredded cheese in the hot bar and sausage and gravy were too warm.

Comboz, 1326 N. Detroit, inspected May 7. Foods in the Pepsi cooler were too warm.

Bambino’s Pizza, 1515 Eleanor, inspected May 7. Both mixers’ undersides were dirty with food residue. Gyro slices were held after expiration.

Wendy’s, 4277 Monroe, inspected May 7. The food prep sink was dirty with food debris and contaminants. No sanitizer buckets were set up during food handling and containers instead were full of dirty water from previous day. The inactive sandwich line on the far side had food containers that were dirty with food debris. Pans of cooked bacon were stacked with food touching the pans’ dirty bottoms. Diced grilled chicken in a sandwich line hot-holding drawer was not hot enough and manager said it appeared to have been left overnight. The active sandwich line cooler failed to hold proper temperature and chopped lettuce, sauces, cut tomatoes, and bleu cheese crumbles inside were too warm. The cooler beneath the fry station failed to maintain proper temperature and two bags of fries inside were too warm. The hamburger patty cooler on the sandwich line failed to maintain proper temperature and raw shell eggs inside were too warm. Salads, an open bag of chopped lettuce, and chopped/​sliced tomatoes lacked required date marks.

Cheese and chicken in the single-door prep-top cooler next to the grill were too warm. A squeeze bottle of pink liquid stored atop the ice machine had no label.

John’s Korner Bar & Grill, 2202 Tedrow, inspected May 9. The ice machine’s slide and ceiling were dirty with black buildup. The ice machine’s drain plumbing had no air gap to prevent sewage backflow.

Flower Drum Restaurant, 49 S. Byrne, inspected May 9. Containers of broccoli in the walk-in cooler lacked covers and were stacked so that container bottoms touched the food.

A Rainbow’s Treasure Childcare & Preschool, 4725 Dorr, inspected May 9. An empty kitchen cabinet had mouse fecies inside.

Waffle House, 6715 W. Central, inspected May 10. Raw shell eggs were stored improperly above ready-to-eat foods. Raw shell eggs were sitting out on a time basis without time markings. Working chemical containers lacked proper labels.

Target, 5225 Monroe, inspected May 10. A carton of raw shell eggs was stored improperly above ready-to-eat food in the walk-in cooler.

Subway, 1564 S. Byrne, inspected May 10. Employees were not verifiably informed of their duty to report illnesses relevant to food safety. The upper part of the self-service drink/​ice machine was dirty with black buildup and specks.

Madison Avenue School of Arts, 1511 Madison, inspected May 13. The person-in-charge failed to ensure employees maintain safe food temperatures during storage or transport and failed to ensure employees wash their hands effectively. An employee touched a garbage bag and then prepared to make sandwiches without washing hands first. An employee stated that she washes her hands in the bathroom instead of at an approved hand-washing sink. No sanitizer bucket was set up. Sandwiches in cold holding were too warm.

Lewis Mart, 4252 Lewis, inspected May 13. Fruit cups offered for sale lacked ingredients labels required for individual sale.

Jamba Juice, 5001 Monroe, inspected May 13. A food handler failed to wash hands between touching a computer screen and making smoothies. Coconut whip topping and Silk milk in the two-door cooler were too warm.

Balance Pan Asian Grille, 5860 W. Central, inspected May 13. Cream cheese filling, lettuce, and chicken in the prep cooler were too warm.

Speedway, 2510 S. Reynolds, inspected May 14. The soda machine’s ice chutes were dirty with brownish buildup. Quaternary ammonium sanitizer dispensed at the three-bay sink was too weak. Nacho cheese and chili in the self-service dispenser had expired. The drink machine’s drain plumbing had an air gap insufficient to prevent sewage backflow.

New Rice King, 1512 Broadway, inspected May 14. The person-in-charge failed to ensure employees are properly trained in food safety, including food allergy awareness. No person-in-charge qualified in food safety was present during inspection. An employee washed hands using the three-bay sink instead of the designated hand-washing sink. An employee touched raw chicken in marinade, then touched cooked chicken without washing hands and changing gloves. Foods in the walk-in and prep-top coolers lacked covers. Chlorine sanitizer solution tested at 0 ppm. No sanitizer was set up for wiping food-contact surfaces. Noodles and chicken and garlic in oil were cold-holding at unsafe temperatures. Food throughout the facility lacked required date marks.
