Kay’s Table, 6600 W. Sylvania, Sylvania, inspected Feb. 18. Food containers had sticker residue on them. The walk-in cooler’s shelves were rusty.

Charlie’s Homemade Ice Cream & Burgers, 6600 Sylvania, Sylvania, inspected Feb. 18. Chemical bottles and rags hung on the hand-washing sink. A paper towel was used to absorb water inside a lettuce pan. Wiping towels were beneath a container of food in the prep cooler by the grill line. There was debris buildup beneath the blender/​mixer’s spindles, inside both ice cream coolers, and on the Hamilton Beach mix and chill (all repeat violations). Dirty dishes in a crate had mold on them (repeat violation). The facility was to be shutting down the first week of March for cleaning with a re-inspection planned the following week.

Shawn’s Irish Tavern, 105 S. Third, Waterville, inspected Feb. 18. Cole slaw, Swiss cheese, shredded cheese, cubed ham, boiled eggs, tomatoes, and hummus in the salad prep-top cooler were too warm.

Mad Anthony’s Tavern, 21 N. Third, Waterville, inspected Feb. 18. An opened package of hot dogs was held past its marked expiration date.

Salad Galley, 3023 Navarre, Oregon, inspected Feb. 19. Pots were filled with water from a hose that was not food-grade (repeat violation).

Toledo Thai Restaurant, 4011 Secor, inspected Feb. 18. Two soap dispensers were stored in the hand-washing sink.

Bubba’s 33, 3534 Secor, inspected Feb. 18. A container of ice was stored improperly in the bar hand-washing sink. Sink & Surface sanitizer in a bucket was not testing at the proper concentration. Housemade garlic butter sitting out at room temperature was too warm. Monte Cristo sandwiches on ice were too warm. No electronic monitoring device was set up in the walk-in cooler where vacuum-sealed foods are stored.

BJ’s Restaurant & Brewhouse, 4905 Monroe, inspected Feb. 18. The kitchen warewashing machine was unable to dispense sanitizer at the proper concentration of 50 to 100 ppm and was testing at 0 ppm (repeat violation). The bar warewashing machine was unable to reach the proper 160-degree temperature for sanitizing on the irreversible thermometer. With multiple tests, the highest reading was 145 degrees.

Baskin Robbins, 4015 Secor, inspected Feb. 18. A green hose was connected to the mop sink faucet without a backflow prevention device to prevent backflow into the clean water supply.

Hooters, 4782 Monroe, inspected Feb. 18. A container of buffalo chicken was improperly reheated. Breaded chicken on the cook line was too warm. Wings were prepped, then placed into the cook line cooler without being chilled first (repeat violation). The person-in-charge did not ensure employees properly maintained safe food temperatures during hot and cold storage. A dishwasher failed to wash hands between handling dirty dishes and clean dishes. The warewashing machine failed to dispense chlorine sanitizer. Drying racks were gritty to the touch. A working chemical spray bottle had no label. Drain plumbing for the bar and soda-station ice bins lacked air gaps to prevent backflow.

Wixey Bakery, 2017 Glendale, inspected. Feb. 19. An employee improperly washed utensils in the hand-washing sink.

Toledo Sylvania Elks #53, 3520 N. Holland-Sylvania, inspected Feb. 19. The dish machine had no chlorine sanitizer (repeat violation).

Gator’s Lounge, 1432 Ingomar, inspected Feb. 19. The ice machine was not clean (repeat violation).

Frontera, 5375 Airport, inspected Feb. 19. Employees entering the kitchen did not wash their hands. Chlorine sanitizer in the dishwasher behind the bar was too weak (repeat violation). A pan of Barbacoa was too warm (repeat violation). Temperature-controlled foods were cooling improperly in deep pans.

City Barbeque, 7402 Central, Sylvania Township, inspected Feb. 19. The ice machine’s interior was dirty with buildup. Milk and ranch dressing in the front cooler were too warm.

The Roadhouse, 11535 W. Central, Swanton, inspected Feb. 19. Small rubber mats were being stored in the front handwashing sink. Chlorine sanitizing solution tested below 50 ppm in the three-bay sink (repeat violation). Coleslaw, chili, and two batches of beans were expired and hadn’t been discarded. Pork was labeled with the incorrect date (repeat violation). Working containers of poisonous or toxic materials were not properly labeled.

Rusty Taco, 1470 Ford, Maumee, inspected Feb. 19. The back prep area hand-washing sink was blocked by three vacuum cleaners and stacks of boxes of cooking oil. An open bag of sugar on the dry storage rack and a bin of dry rice with no lid on the shelf below the back work table were exposed to contamination. The ice machine’s interior was dirty with buildup. The can opener’s cutting surface was dirty with dried food residue. Quaternary ammonium sanitizer in the cooking area wipe-cloth bucket was too weak. Cups of salsa on ice for self service at the front counter was too weak (repeat violation). Refrigerated sauces, including ranch dressing, chipotle ranch, and achiote sauce, were held past their expirations. A chemical spray bottle had no label (repeat violation). A stainless steel bowl used for food preparation was badly dented and damaged, making it difficult to clean.

Toast and Tap, 413 Madison, inspected Feb. 20. A package of raw steak sat on top of a container of cooked chicken. Red cabbage sat unprotected outside on top of the shipping box.

Shorty’s True American Roadhouse, 5111 Monroe, inspected Feb. 20. The bar hand-washing was used improperly to hold soapy water for cleaning the bar area. The bar’s chemical dish machine was not sanitizing dishware.

Ohio Living - Swan Creek, 5916 Cresthaven, inspected Feb. 20. The bottom of the ice scoop holder was filled with debris. Egg salad and tuna salad were too warm, holding at 45 degrees in the skilled nursing kitchen (repeat violation).

McDonald’s, 3005 N. Holland-Sylvania, inspected Feb. 20. The ice machine’s interior was dirty with buildup.

Fairways, 8256 W. Central, inspected Feb. 20. Quaternary ammonium sanitizing solution was too weak. The cutting board was dirty with buildup (repeat violation).

Cinco de Mayo Amigos, 6975 W. Central, inspected Feb. 20. A speaker and a dry rag were stored on top of the hand-washing sink faucet. The ice machine’s interior was dirty with buildup (repeat violation). Chlorine sanitizing solution in the dish machine tested at 0 ppm (repeat violation). Rice sitting on top of the steam well was holding at an unsafe temperature and chicken and beef were in a well without the heat turned on. Temperature-controlled foods lacked proper date marks (repeat violation). Chemicals were stored above food-contact items. A cutting board was no longer easily cleanable.

B.M. Chen, 1227 S. Reynolds, inspected Feb. 20. Sweet and sour chicken was not vented for cooling.

Sylvania Moose Lodge No. 1579, 6072 N. Main, Sylvania, inspected Feb. 20. The bar glass washer did not dispense sanitizer when required. Items in the prep cooler had no date markings.

J&G Pizza Palace, 5692 N. Main, Sylvania, inspected Feb. 20. Washed containers and lids were dirty with sticker residue.

Local Thyme, 6719 Providence, Whitehouse, inspected Feb. 20. Turkey in the bottom of the prep-top cooler was held past its marked expiration date.

China One Buffet, 3040 Navarre, Oregon, inspected Feb. 20. The three-bay sink was set up in a wash-sanitize-rinse order, which is incorrect.

Reynolds Mart, 1027 N. Reynolds, inspected Feb. 20. The microwave oven was dirty with buildup and debris. No sanitizer was set up during food handling. Acidified bathroom cleaner was on the counter near the cappuccino machine.

The Fieldhouse, 1040 N. Westwood, inspected Feb. 21. Chlorine sanitizer in the upstairs dish machine was too weak.

Tasty Thai, 1515 S. Byrne, inspected Feb. 21. Consumers were not properly advised of the risk of consuming raw or undercooked animal foods.

University of Toledo - Savage Arena Suites Kitchen, 2801 W. Bancroft, inspected Feb. 21. Chicken in a hot holding cabinet was not hot enough (repeat violation).

University of Toledo - Savage Arena #2, 2801 W. Bancroft, inspected Feb. 21. Nacho cheese in a dispenser was not warm enough. Hot dogs in the prep-top cooler were too warm, perhaps because the cooler was overfilled.

Rose Thai, 5333 Monroe, inspected Feb. 21. Food in the walk-in cooler lacked covers.

EZ 4, 868 East Broadway, inspected Feb. 21. Eggs were stored improperly above other food items. The three-bay sink had no hot water.

Classic Lounge, 2224 Nebraska, inspected Feb. 21. Milk in the bar cooler was too warm.

Damascene Grill, 5834 Monroe, Sylvania, inspected Feb. 21. A pan of medicinal water for an employee’s personal use blocked access to a hand-washing sink. Raw beef was stored improperly above alcohol bottles and juices. Large bags of rice and salt were uncovered. Pita bread and pita chips beneath the prep table shelves were uncovered. Dish racks were dirty with excessive buildup.

China King, 1083 Pray, Waterville, inspected Feb. 21. The container and lid used for storing cabbage in the walk-in cooler were both visibly dirty. There was no chlorine bleach sanitizer in the wipe cloth bucket. The large tub containing broccoli at the cookline area was damaged and had a hole in it.

China House, 2112 N. Holland-Sylvania, inspected Feb. 21. Raw shell eggs were stored above pork in the walk-in cooler (repeat violation). The slicer was dirty with buildup (repeat violation).

Bier Stube, 5333 Monroe, inspected Feb. 21. A five-gallon tub of french fries that had been blanched the previous night was cooled improperly and was too warm. Housemade Samurai sauce and Redhead sauce were held past their marked expiration dates.

Adams Pizza & Wings, 1032 N. Holland-Sylvania, inspected Feb. 21. Salad mix was in a strainer in the hand-washing sink. Flour containers were dirty with buildup (repeat violation). Chlorine sanitizing solution in the wipe bucket was too weak. Temperature-controlled foods had incorrect expiration dates.

S&G, 10165 Airport, Monclova, inspected Feb. 21. Quaternary ammonium sanitizing solution tested at 0 ppm. Clorox wipes, antifreeze, and motor oil were stored above food items on a storage rack.

Maumee Bay State Park Lodge, 1750 Park, Oregon, inspected Feb. 24. Chlorine sanitizer in the bar area dish machine was too weak.

X-Golf Toledo, 7141 Spring Meadows West, Holland, inspected Feb. 24. Portioned cups of pork in the bottom of the prep-top cooler had expired (repeat violation).

LaVie Mediterranean Grill, 1572 Spring Meadows, Holland, inspected Feb. 24. A waitress put lemon slices in customer drinks with her bare hands. A waitress rinsed spoons in the cookline hand-washing sink. Raw shell eggs were stored improperly above ready-to-eat foods in the walk-in cooler. The potato cutter was dirty with buildup (repeat violation). Hummus was dated incorrectly. Chemical spray bottles lacked proper labels (repeat violation).

Sam’s Club, 1300 East Mall, Holland, inspected Feb. 24. The high-temperature dish machine did not get hot enough to sanitize wares.

Twin Peaks Restaurant, 5060 Monroe, inspected Feb. 24. A food handler dumped a sanitizer bucket into the hand-washing sink. Sanitizer in the wipe bucket and from the warewashing dispenser was too weak. Cooked jalapeno peppers in hot holding were not hot enough. Housemade garlic butter sat out at room temperature and lacked a required time stamp.

The Food Station, 2051 Dorr, inspected Feb. 24. Temperature-controlled, ready-to-eat foods including cut leafy greens, cut tomatoes, hard boiled eggs, open tzatziki sauce, feta cheese, mozzarella cheese, Colby jack cheese, turkey bacon, cooked pasta, and cooked broccoli lacked date marks. Employees were not verifiably informed of their duty to report illnesses relevant to food safety. The food prep sink’s drain plumbing was not properly air gapped to prevent backflow.

TolHouse, 1447 N. Summit, inspected Feb. 24. The deli slicer in the prep room was not being cleaned at the required times during and after use. Dried protein buildup was underneath the blades and parts of the slicer in the prep room (repeat violation). Diced fried potatoes and sliced tomatoes were not cold enough due to being left sitting out on the prep cooler in the kitchen. Temperature-controlled foods in the two-door True cooler in the prep room and in the walk-in cooler lacked proper date marks. Guacamole, cooked rice, boiled eggs, and soup in the two-door Avantico cooler and cooked potatoes in the walk-in cooler lacked date marks.

Mr. Gyros & More, 3833 Airport, inspected Feb. 24. No person-in-charge certified in food safety was present during inspection.

Byblos, 1050 S. Reynolds, inspected Feb. 24. The ice machine plate was dirty with buildup. Soups and other temperature-controlled foods lacked date marks.

Bar 145, 5305 Monroe, inspected Feb. 24. The hand-washing sink by the back prep sink was blocked by multiple strainers hung in the way (repeat violation). Drying racks were gritty and tacky. The french fry press had food residue on it. Buffalo chicken dip had not been reheated properly and was not hot enough. Cut tomatoes in the far right make table were too warm and the person-in-charge could not explain it. Drain plumbing for the ice machines, bar ice bin, and server station ice bin lacked air gaps to prevent backflow.
