Fresh Thyme Market, 5105 Monroe, inspected Aug. 2. The produce case holding cut melon, guacamole, and vegetable dip failed to maintain proper refrigeration temperature.

Mancy’s Restaurant #2, 953 Phillips, inspected Aug. 3. A box of oysters lacked a proper shellstock tag identifying the product’s source. Eggs and egg products were stored improperly above other items in the walk-in cooler. The slicer was dirty with food debris. Washed containers in the clean dish rack had sticker residue and dirty film. Beef stock in large containers in the walk-in cooler were cooled improperly and holding at an unsafe temperature. Cut leafy greens lacked proper date marks. Chemical spray bottles hung on dry-storage shelves near food. Raw meats were packed in reduced-oxygen packaging and sous vide food preparation was occurring, for which proper documentation in place for neither. The facility was infested with flies.

Mancy’s Restaurant #1, 953 Phillips, inspected Aug. 3. Shellstock tags for some oysters were not properly marked with the safe-use date. An uncovered ice cream cake was in direct contact with containers stored next to it in the freezer. Oil in a pan used to brush on potatoes was left uncovered in the potato room. Towels were placed improperly beneath cutting boards on the main cook line. The upstairs bar’s pop guns and holsters were dirty with slime buildup and the ice bins were dirty with debris and slime. The iced tea dispenser and nozzle at the back wait station were dirty inside with slime buildup. A dirty single-use glove was left on the surface of cleaned and sanitized dishware. The ice machine’s interior was dirty with slime buildup. Multiple bins used for dispensing salad from the salad cooler were left dirty on a floor and one had an employee’s purse in it. The ice bin at the upstairs back wait station was filthy with old food and debris. Two drawers of potatoes in the potato warmer were holding at an unsafe temperature. Watermelon vinaigrette was held past its marked expiration date. A chemical spray bottle was stored with potatoes in the potato prep room. Sanitizer spray bottles were stored improperly on the food prep sink’s drainboard. Raw meats were being packed in reduced-oxygen packaging without proper documentation and without proper date marks. Repeat violation. Mouse droppings were in the back stock room and live cockroach activity was observed during inspection. Spatulas, pan lids, food containers, food container lids, steam table lids, and scrapers had chips, cracks, holes, and other damage making them difficult to clean.

La Fiesta Toledo, 2500 W. Sylvania, inspected Aug. 3. Pests were present both upstairs and downstairs. A hose was connected to the three-bay sink’s faucet without adequate backflow prevention in place.

Krieger’s Pub, 2150 W. Laskey, inspected Aug. 3. The hand-washing sink was used improperly as a dump sink.

Great Black Swamp Brewing Co., 2248 Tedrow, inspected Aug. 3. The ice machine and ice bin drains lacked air gaps in their plumbing.

Burger King, 4010 Monroe, inspected Aug. 3. Flies were present.

Berger’s Olde Tyme, 1742 W. Laskey, inspected Aug. 3. Refrigerated foods lacked proper date marks.

Sip & Brew, 1923 W Alexis, inspected Aug. 4. Hollandaise sauce was improperly left out of refrigeration.

Scramblers, 6819 W. Central, inspected Aug. 4. Foods in both levels of the cook line prep cooler were holding at unsafe temperatures. Raw shell eggs left out for prep were holding at an unsafe temperature.

River East Cafe, 1316 Front, inspected Aug. 4. Various items were improperly placed in and in front of the hand-washing sink.

Jed’s On Campus, 3534 Dorr, inspected Aug. 4. Buildup was developing inside the ice machine.

Upton Express, 3645 Upton, inspected Aug. 5. Employees had not been verifiably informed of duty to report illnesses relevant to food safety. The hand-washing sink was full of items. The pop fountain’s ice chute was dirty with slime.

Mr. Freeze, 2031 N. McCord, inspected Aug. 5. The chlorine sanitizer in a wipe bucket was at the wrong concentration. Live flies were present.

Health Foods By Claudia, 3904 Secor, inspected Aug. 5. Sample coliform-test results for the bulk water machine were not available.

Star Diner, 6710 W. Central, inspected Aug. 5. A large trash can blocked access to the hand-washing sink. The slicer was dirty. Milk in the server cooler was holding at unsafe temperatures. Live gnats were present.

Buffalo Wild Wings Grill & Bar, 6710 W. Central, inspected Aug. 8. A stand-up fan blocked access to the cook line hand-washing sink. The prep cooler by the hand-washing sink did not maintain safe refrigeration temperature.

Bagel Place, 4024 N. Holland-Sylvania, inspected Aug. 8. A bucket in the food production room’s hand-washing sink blocked its proper use.

Ventura’s, 7742 W. Bancroft, inspected Aug. 9. The server ice machine’s drip edge was dirty with buildup. Chlorine sanitizer for the dish machine was not primed properly and did not dispense.

The Original Gino’s Pizza, 1280 W. Alexis, inspected Aug. 9. An employee failed to wash hands before working with food. The dough mixer was covered in filth. Multiple items in the walk-in and sandwich coolers lacked date marks.

S&G, 505 Eleanor, inspected Aug. 9. The person in charge failed to demonstrate proper knowledge of cleaning and sanitizing or to ensure employees properly sanitized equipment and utensils. The person in charge failed to ensure employees are properly trained in food safety. Employees had not been verifiably informed of duty to report illnesses relevant to food safety.

Poco Piatti, 6710 W. Central, inspected Aug. 8. The person in charge cut ready-to-eat cucumbers without wearing gloves. A dish rack blocked access to the kitchen hand-washing sink. A strainer was placed improperly atop the bar hand-washing sink’s basin. Several food items in the walk-in cooler and dry stock area, as well as flour for bread in the customer area, lacked covers. The ice machine’s interior was dirty with buildup. Chicken, grape leaves, and feta dip in refrigeration lacked proper date marks. A chemical spray bottle had no label.

So Sweet Lebanese French Pastries, 4038 Talmadge, inspected Aug. 9. Employees had not been verifiably informed of duty to report illnesses relevant to food safety. Foods in the walk-in cooler and on storage racks lacked covers. Milk and white chocolate mousse in the walk-in cooler and rice pudding in the display cooler were holding at unsafe temperatures. Cut cantaloupe and containers of mousse in the walk-in cooler lacked proper date marks. Small propane tanks were stored next to containers of simple syrup in the walk-in freezer. The KitchenAid mixer paddles had chipping paint and a wooden spoon in poor condition was in use.

Midtown Food Mart III, 6215 Hill, inspected Aug. 9. Raw shell eggs were stored improperly above ready-to-eat foods in the walk-in cooler.

Boil Shack, 4038 Talmadge, inspected Aug. 9. The cook changed gloves several times without washing hands. Foods in the walk-in cooler and in the kitchen lacked covers. Raw chicken wings were stored improperly above corn in the walk-in cooler. Cooked chicken was stored improperly in a used concrete box inside the prep-top cooler. An employee’s water bottle was stored improperly in the edible ice in the ice machine; ice used as a coolant may not be used as food.

S&G, 1505 Eleanor, inspected Aug. 10. Cappuccino machine components were dirty with buildup. Gnats were present in the three-bay sink and mop sink.

Merit House, 4645 Lewis, inspected Aug. 10. A toaster was dirty with buildup.

Shawn’s Irish Tavern, 7436 W. Bancroft, inspected Aug. 9. Deli meats were stored improperly on top of packaged raw steak. The quaternary ammonium sanitizer dispenser at the three-bay sink was not primed properly and thus did not dispense sanitizer into the wipe buckets.

Kroger, 4533 Monroe, inspected Aug. 10. A bakery employee had long fingernails that were dirty underneath. Raw turkey sausage products were stored improperly above other raw sausage products. Food containers in the sushi kitchen were dirty with food debris buildup. The sushi kitchen’s cutting boards were dirty and stained. The bottom of a cutting board and the top of the sushi kitchen’s two-door cooler upon which it sat were filthy. Cooked rice in a deep, covered plastic container in the prep-top cooler was cooled improperly and was holding at an unsafe temperature. A meat and seafood display cooler did not maintain safe refrigeration temperature. Alternative milks and juices in the produce-prep walk-in cooler were holding at unsafe temperatures. The walk-in cooler’s door was left open, impairing its capacity to refrigerate. Chemicals and foods were stored together improperly in a clearance area. The sushi kitchen area was cluttered and disorganized and storage racks were too low to the floor to allow cleaning. Gnats and flies were present in the bakery and deli areas. The deli area’s insect light did not work and trash, including molding fruit, was stored in the bakery area.
